This is the performance team
Elite dancers perform under the Sensual Movement name.
Look at the current team before you write the last box — it is the clearest picture of what is being asked for.

Two teams, two forms
The elite dancers and the artist roster are different groups, and they take applications separately.
Teaching, not performing?
The artist roster is the instructors and performers listed under Our Artists.
Nearly the same questions, a different team. Apply there if that is the one you want.
Artist application →Looking for the paid programme?
The Artist Development Program is a one-year mentorship at $3,200 per person.
Neither of these two forms is that. It has its own page and its own application.
